When we auscultate, we hear korokoff sound (maybe wrongly spell). Those sound indicate systolic and diastolic blood pressure. When you first heard korokoff sound, it’s a systolic reading on sphygnomanometer but when the sound fade until you can’t hear, it’s a diastolic reading. You inflate higher than systolic BP because you want to hear the first korokoff sound to get the accurate systolic BP reading, if you inflate lower than systolic BP, when you auscultate, you can’t detect the 1st korokoff sound which like I said before indicate systolic BP reading.
To inflate high BP patient is same with other patient. Palpate the pulse and inflate until you felt the pulse disappearing. At that level you know there is no more blood flowing at that time. When you deflate, the first sound heard is sytolic BP reading.
